
New Delhi: Diagnostics service provider Redcliffe Labs has reported the diagnosis of a subcutaneous tailgut cyst in a 37-year-old woman, marking an unusual presentation of a typically deep pelvic congenital condition.
The case, led by Dr. Mayanka Seth, Consultant Pathologist at Redcliffe Labs, has been published in the Indian Journal of Case Reports.
Tailgut cysts are rare developmental anomalies arising from embryonic gut remnants. While they are generally located deep in the pelvis, according to the company, this case involved a cyst near the tailbone just beneath the skin—an uncommon manifestation that led to years of misdiagnosis.
It is indicated that the patient had experienced a painful, discharging lesion in the sacrococcyx region for over eight years and underwent multiple surgical drainages with no lasting relief. Initial clinical impressions pointed to common conditions like an infected sinus or abscess. However, further evaluation revealed atypical characteristics, leading to surgical removal of the lesion.
Histopathological examination confirmed the presence of a tailgut cyst. Following the complete excision, the patient reported significant symptom relief and improved quality of life.

In this case, the lesion had been misdiagnosed and left untreated for years. It highlights the importance of considering rare possibilities, even in patients without obvious symptoms.
A correct diagnosis often depends on the right combination of clinical judgment and pathology. When used together, they can guide timely decisions and lead to effective treatment, especially in complex or persistent cases.”
